Complete our online referral form and our referral team will contact you within 48 hours to schedule your appointment. You may also click on the forms tab under resources and complete our referral form. Once the form has been completed you can email the form to support@mytherapyworld.com , fax it to (803) 228-0101, or mail it to the following address:

Once the referral form has been received, our referral team will contact you to schedule your appointment. 

While setting up your appointment, our referral team will contact your child’s primary pediatrician and request a physician order to be signed. 

YES! We offer free 30-minute screenings in our clinic. Please call our office at (803) 329-9500 to schedule your child’s FREE screening. 

We recommended that you complete the in-take packet form that we will email to you prior to your appointment. You may also find the form by clicking on the forms tab under resources. You can fill the document electronically or print it out. You must bring in a copy of your insurance card and any medical history documents that may be important for the evaluating therapist to review. Suggested documents that you can bring to your child’s evaluation are: Individualized Family Service Plan (IFSP), Individualized Education Plan (IEP), diagnosis reports, and surgical or hospitalization reports, just to name a few. 

Therapy World has a talented team of billing specialists that are trained to work with you to contact your insurance and determine the costs of therapy sessions.
